INSTRUCTIONS

This class action alleges VIZIO violated privacy and consumer-protection laws by collecting viewing data from Smart TVs for sale to advertisers. VIZIO denies these allegations.

You are a Settlement Class member if you are:

An individual in the United States who purchased a VIZIO Smart Television for personal or household use, and not for resale, that was subsequently connected to the Internet at any time between February 1, 2014 and February 6, 2017.


Only one purchaser per Smart TV per household may qualify. Anyone who purchased more than one qualifying Smart TV should include each qualifying Smart TV with their claim submission.


To be eligible for payment you must submit a valid claim no later than April 29, 2019.


Settlement payments will be digitally sent to you via email. Please ensure you provide a current, valid email address and mobile phone number with your claim submission. If the email address or mobile phone number you include with your submission becomes invalid for any reason, it is your responsibility to provide the accurate contact information to the Settlement Administrator to receive a payment. When you receive the email and/or mobile phone text notifying you of your Settlement payment, you will be provided with a number of digital payment options to immediately receive your settlement payment. You will also at that time have the option to request a paper check.

The information provided on this Claim Form will be used solely by the Court-approved Settlement Administrator for the purposes of administering the settlement and will not be provided to any third party or sold for marketing purposes.
